Releasing Confidential Information

Your confidential information can be released by signing a form which states the name of the person who you want to read it, the purpose for which you expect them to use it, the date, and the length of time you expect that person to use it. On the following forms, you will find releases of information which you will be asked to sign:

• Consent for One-Time Release of Information - This form must accompany any documentation forwarded to the Center for Accommodative Services
• Consent for Periodic Release of Information
• Consent to Release Information-Legal Form
