Physicals

Entry physicals should be done prior to arrival at Paul Smith’s College. Student Health Services will accept a physical that is within one calendar year from the student’s date of entry.

Please upload all physical paperwork to the Paul Smith’s Health Portal. You may either print out the Health Services Physical form, and upload that to the portal once complete; or we will also accept an uploaded copy of the visit note from the doctor.

Entry physicals are not covered under our contract and, therefore, are not provided at the Paul Smith’s clinic. If a physical must be obtained after arrival, Mountain Medical in Saranac Lake performs physicals.  Please see their website for more details – Main | Mysite (mountainmedical.net).

Students are required to have proof of insurance and are required to cover the cost of the physical.
